| 内容記述 | 今日、高齢者福祉において「介護保険」は欠かせない制度となっている。2000年4月の全面施行から20年を経た現在は、2021年4月から始まる2023年度までの新たな介護保険料の改定、第7期介護保険事業計画に向けた方針のもと、全国1700余の自治体でその準備が進んでいる。今後、団塊世代が後期高齢期に入り高齢化率がピークに達する社会では、同制度は欠かせない基幹であり、期待はきわめて高い。もともと介護保険は1963年の老人福祉法に続き、急速な人口の高齢化に向け高齢化が進む社会のために準備された。自立支援、在宅での福祉をコンセプトに導入を進め、地方自治体は制度運営の保険者として責任を担う大きな転換点になった。本論は、制度がスタートした2000年4月の制度導入までの経緯を振り返り、最前線の自治体である東京・世田谷区での地域福祉の取り組みを考察した。自治体では、それまでの福祉やコミュニティ施策を総動員し介護保険を契機として、地域における福祉を大きく進めた。直下では、新型感染症のコロナウイルスの流行抑止のため公共施設の使用制限や地域福祉・コミュニティ活動が制約されるなど厳しい状況が続いている。こうした時期に、改めて介護保険スタートを振り返る。 | |||||||||||

| 内容記述 | Nowadays long-term care insurance (LTC) plays an essential role in the welfare system for the elderly. Twenty years after its nationwide implementation in 2000 April, more than 1700 municipalities are currently preparing for the seventh revision of the LTC policy that covers the new LTC premium between 2021 April and 2023 March. Facing the unprecedentedly aging society due to the elderly baby-boomers, LTC is expected to play the pivotal role in the country. Following the Act on Social Welfare for the Elderly in 1963, LTC was originally planned so as to cope with the rapidly aging population. Aiming for “welfare at home” and “supports for the elderly independence”, the municipalities became the insurers that would take more responsibilities in the elderly welfare by the LTC implementation. This article reviews development of LTC planning before its implementation in 2000 April and discusses the actions taken in Setagaya, Tokyo, as an example of municipal welfare. The local political body drove the local welfare project forward based upon the existing welfare system and previous communal policies, which was triggered by the LTC implementation. The local welfare/communal actions are restricted at the moment for the limited use of public facilities due to the COVID-19 pandemic. The article revisits the implementation of LTC that is currently going through difficulties. | |||||||||||
